Major cleanup of the FRAME, CONTEXT, and FONT abstraction layers.

The PFRAME_DESC, FRAMEPTR, PCONTEXT_DESC, CONTEXTPTR, PFONT_DESC, and 
FONTPTR types are all now defunct.  gfxintrn.h has also been vastly 
simplified.
